This is the clinic of Dr William Keen, shown on the right (with the beard). He was a Marshall of American Surgery & Pioneer of Neurosurgery. This surgeon was America's first brain surgeon. He was the first person to remove a brain tumor. He wrote a number of books including Gray's Anatomy. This image was taken just a few years before his retirement.
The room we are looking at is known as an operation theater. This is a learning hospital. Naturally due to his skills as a doctor, many want to see his operations. It's a full house, most of the seats are filled, and people are forced to stand there with the patient.
Do note how close people are to the patient, along with the lack of gloves and masks. In these days they really didn't know about keeping the area clean, the best they had to offer was white clothing to make it look clean.
